Job Summary

Develops and administers physical therapy Plan of Care for patients to restore function, relieve pain, and prevent disability following illness, disease, or injury.

Patient Rights
• Ensures that care is provided in privacy
• Ensures protected health information is kept confidential
• Reports complaints made by patients to supervisor
• Reports allegations of patient abuse, neglect and/or misappropriation of patient property

Safety and Sanitation
• Follows established safety and infection control policies and procedures
• Observes safety needs of patients as indicated in plan of care
• Wears and/or uses safety and personal protective equipment and/or supplies when indicated and as properly trained to use
• Demonstrates job-specific knowledge of fire and disaster preparedness during drills or actual situations

Staff Development
• Attends and participates in scheduled in-service training, educational classes and meetings to maintain current credentials as applicable and as managed by regulatory agencies and company policies.
• Participates in QAA program
Physical Therapist Responsibilities
• Reviews and evaluates physician’s referral and patient’s medical records to determine physical therapy treatment required.
• Plans and prepares written plan of care based on evaluation of available patient data.
• Confers with physician and other health practitioners to obtain additional patient information, suggests revisions in treatment programs, and integrates physical therapy treatment with other aspects of patient care. • Records patient treatment, response, and progress.
• Assists the Clinic Director in planning of physical therapy services, as needed.

• Performs tests, measurements, and evaluations, such as functional activities/ADLs, range of motion, manual muscle tests, gait analysis, and records and evaluates findings to aid in establishing or revising Plan of Care; • Provides evaluation and administers a variety of treatments to the musculoskeletal, neuromuscular, cardiopulmonary and integumentary systems.
• Evaluates effects of treatments at various intensities and durations and adjusts treatment to achieve maximum benefit.
• Instructs, motivates, and assists patients in exercises, such as isometric and progressive-resistive and in functional activities, such as gait training, transfer training; Utilizes weights, exercise machines, mats steps, inclined surfaces, assistive and supportive devices such as walkers, canes, crutches, parallel bars, orthoses, and prostheses.
• Consults with physician in the advent of adverse reactions.
• Maintains documentation consistent with third party reimbursement.
Supervisory/People Management Responsibilities
• Provides input to the Clinical Director on performance appraisals for Physical Therapist Assistants and Rehabilitation Aides
• Delegates and supervises appropriate clinical responsibilities according to their State practice act.

Administrative Responsibilities

• Participates in department meetings and other meetings as assigned by Clinical Director.
• Follows H2 Health Policies and Procedures
• Maintains confidentiality of necessary information.
• Performs any miscellaneous work assignments as may be required.
• Assists the facility in assuring adherence to Federal and State regulations and certifications.

Education
• Meets the educational requirements by being a graduate of an accredited physical therapy program for Physical Therapy and obtaining a passing grade on the National Physical Therapy Examination or currently licensed in the state where you are practicing.

Experience
• Prior experience in Physical Therapy, preferred.

Physical Demands
• Lifting 50 pounds maximum with frequent lifting and or carrying of objects weighing up to 25 pounds. Walking, standing, bending, stooping and sitting for prolonged periods. Full use of gross and fine motor skills of the upper and lower extremities. Possesses verbal and written communication skills. Ability to use senses in applicable tests and equipment use.

Travel
• May provide services in a patient’s home or contextual environment including but not limited to a physician appointment, in-home intervention or home assessment, assist at other centers and/or attend company sponsored meeting.

Working Conditions
• Inside with exposure to some disagreeable conditions and sometimes outside contracted events.

Principal Contacts
• Patients/visitors and all co-workers.
